Reimagine The Way You Provide Healthcare

Learn more about us.

CHI offers integrated clinical service coordination to healthcare systems and physicians. We also provide practice support coordination for physicians and mid-level providers, often aligning these services with our clinical coordination efforts. Our core mission is to ensure continuity of care across the healthcare system. This is possible because our providers are credentialed at multiple hospitals, clinics, and physician networks. As a result, patients may encounter a familiar provider across different settings, allowing us to seamlessly coordinate care. With appropriate consent, we can consult with a patient’s other providers to support their treatment.

The providers we match with our clinical service coordination clients are trained and credentialed by those clients. We ensure they stay integrated within the system by providing continuous updates on training and facility-specific changes. This keeps them informed of any shifts in practice style or clinical protocols, ensuring they remain aligned with the evolving needs of the facility and the physicians they support.

Our clients are medical and psychiatric healthcare providers. We serve hospitals, clinics, physicians, mid-level providers, and healthcare systems looking for integrated clinical service coordination and practice support. Our services are designed to help these providers improve patient outcomes, streamline operations, and enhance care delivery by offering flexible care, integrated updates & training, and seamless coordination across multiple care settings.
